What PCOS Is, and How It Differs From PCOD

PCOS is diagnosed when two of the three Rotterdam features are present: irregular or absent periods, raised androgens on blood tests or their signs, and polycystic ovaries on ultrasound.

PCOD only describes polycystic-looking ovaries on a scan. Plenty of women have ovaries like this without the hormone changes of PCOS, and if your periods are regular and your hormones are normal, you may need no treatment at all. PCOS is the diagnosis that needs active care.

In May 2026 a global consensus published in The Lancet renamed the condition polyendocrine metabolic ovarian syndrome, or PMOS. This page uses PCOS, the name most people know.

Signs That Point to PCOS

Irregular or missed periods, with cycles longer than 35 days, together with signs of raised male hormones are the usual pattern.

  • Irregular periods that come rarely or stop altogether
  • Excess facial and body hair on the upper lip, chin, chest or abdomen
  • Hormonal acne along the jawline, chin and back
  • Thinning hair at the crown and temples
  • Weight gain around the waist
  • Dark, velvety skin at the neck, armpits or groin
  • Low mood, anxiety, tiredness or trouble concentrating
  • Difficulty getting pregnant

One sign on its own does not make the diagnosis, so the next step is a proper assessment.

Tests at Your First PCOS Visit

Blood tests for hormones and insulin, plus a pelvic ultrasound, are the usual first tests, and Dr. Rizwan chooses which ones you need from your symptoms and history.

Common PCOS tests
TestWhy it is done
LH and FSHTo assess the signals between the pituitary gland and the ovaries
Total and free testosteroneTo measure androgens, the male hormones behind excess hair and acne
DHEASTo check androgens made by the adrenal glands
AMHTo assess ovarian reserve, the supply of eggs
Fasting insulin and glucoseTo look for insulin resistance and raised blood sugar
Thyroid function testsTo find thyroid problems, which are common alongside PCOS
ProlactinTo rule out high prolactin, another hormonal cause of irregular periods
Pelvic ultrasoundTo look for many small follicles on the ovaries

The results are read with a detailed clinical history before any treatment is suggested.

How PCOS Treatment in Lahore Works

There is no single best medicine for PCOS: the plan follows your results and your main goal, and it usually starts with diet and activity changes that improve insulin sensitivity.

Insulin resistance drives much of PCOS. When cells respond poorly to insulin, the body makes more of it, which makes fat easier to store and pushes the ovaries to make more androgens. That is why weight can be so hard to shift, and why the risk of type 2 diabetes is higher. Even a 5% loss of body weight can help periods and ovulation work better.

If irregular periods or acne bother you most and you are not trying to conceive, combined hormonal contraceptives are usually the first medicine, after a check of your blood pressure, smoking and clotting history. Metformin is used where insulin resistance or blood sugar is the main problem; it can help irregular periods for some women, but it is not reliable for excess hair or acne, so medicines for those are added when they bother you most. The same clinic provides diabetes care in Lahore if sugar has already risen. Thyroid treatment is added when the tests call for it.

For weight-focused treatment, Dr. Rizwan discusses Mounjaro injections after a careful assessment, and his article on why weight loss is hard with PCOS goes further.

Please do not self-medicate: the wrong medicine can make the hormone imbalance worse. Follow-up visits recheck hormones and metabolic markers, adjust the plan and catch early signs of diabetes. If you live outside Lahore, you can book an online consultation.

Can You Get Pregnant With PCOS?

Most women with PCOS can conceive. Irregular ovulation is the obstacle, and bringing the hormones and metabolism under control often restores it.

PCOS is the most common cause of infertility from absent ovulation, and also one of the most responsive to treatment. A good number of women conceive without any fertility procedure once their hormone and metabolic health is under control. When pregnancy is the goal, ovulation is induced under close supervision with medicines such as clomiphene or letrozole, and it is monitored. Women with PCOS who conceive have a slightly higher risk of gestational diabetes, so blood sugar is watched during pregnancy. The page on hormonal causes of infertility goes into more detail.

Why See an Endocrinologist for PCOS Treatment in Lahore

The hormones and metabolism behind PCOS are an endocrinologist's field, while IVF and other fertility procedures belong to fertility clinics.

An endocrinologist reads the hormone and metabolic results together and follows them over time, so one doctor sees the whole picture.

Frequently Asked Questions

Can PCOS be cured permanently?

There is no permanent cure in the usual sense, but PCOS can be managed very well. Many women reach regular cycles, normal hormone levels and a good quality of life with treatment, and for some, significant weight loss brings near-complete relief. PCOS-related hormone symptoms usually settle after menopause.
