Parathyroid Conditions We Treat
The four tiny parathyroid glands sit behind the thyroid and regulate blood calcium. Disorders here affect bones, kidneys, muscles and nerves.
Primary Hyperparathyroidism
One or more overactive parathyroid glands produce excess PTH, raising blood calcium. Causes kidney stones, bone loss, fatigue, and depression. Often found incidentally on blood tests.
Secondary Hyperparathyroidism
PTH rises as a compensatory response to low calcium – most commonly from vitamin D deficiency or chronic kidney disease. Treating the underlying cause is key.
Hypoparathyroidism
Low PTH causes low calcium (hypocalcaemia), leading to muscle spasms, tingling, and seizures. Often occurs after thyroid or neck surgery. Requires calcium and vitamin D supplementation.
Hypercalcaemia
High blood calcium is most often caused by hyperparathyroidism. Symptoms include excessive thirst, frequent urination, constipation, fatigue, and confusion in severe cases.
Diagnosis & Treatment
Parathyroid disorders are diagnosed with straightforward blood tests. Treatment depends entirely on the type and severity.
Blood Tests
Serum calcium, albumin-adjusted calcium, intact PTH, phosphate, 25-OH Vitamin D, and kidney function assessed together to determine the type of parathyroid disorder.
Bone Density Scan
DEXA scan assesses bone mineral density loss caused by chronic PTH excess. Guides decisions on monitoring vs surgical referral.
Medical Management
Secondary hyperparathyroidism is treated with vitamin D and calcium correction. Hypoparathyroidism requires calcium and activated vitamin D supplementation with careful monitoring.
Surgical Referral
Primary hyperparathyroidism with significant hypercalcaemia, kidney stones, or bone loss is referred for parathyroidectomy. Dr. Rizwan Niazi provides pre- and post-operative hormonal care.
