You opened the report and saw a percentage that did not come with a simple answer. Here is the direct reply. HbA1c is a blood test that shows your average blood sugar (blood glucose) over the last 2 to 3 months because red blood cells live about that long. If you want a quick reading, that percentage tells me how much sugar has been sticking to the haemoglobin in your blood over the previous weeks, not what your meter read this morning.

What does HbA1c tell you?
The simplest way I explain it in clinic is this. Sugar sticks to haemoglobin in your red blood cells, and the lab measures the percentage that has sugar attached. That percentage is your HbA1c (the medical name in brackets) and it acts as a three month memory of average blood glucose. I use it to see the trend, not the single high or low that you may have seen on your meter.
One pattern I see almost every week is patients who say, “My fasting sugar is fine, so why is my report still bad?” The answer lies in the difference between a single point and an average. If you spike after meals several times a week, the meter and the A1c tell two complementary parts of the story.

What is a normal HbA1c?
Here are the ranges I give patients so they can read their own report without guesswork. A result below 5.7% is considered normal. A result between 5.7% and 6.4% places you in the prediabetes range. A result of 6.5% or higher is in the diabetes range. Those cutoffs are the ones I use when screening, not a judgement about you as a person.
Most of my patients are surprised when I tell them that these numbers are a way to decide whether we need more tests or a treatment plan. If your report sits in the prediabetes band, it is a signal to act now so the number does not creep higher.
Is 6.5 HbA1c diabetes?
Short answer. A single HbA1c of 6.5% or higher usually falls in the diabetes range, but I often ask for confirmation. In people without symptoms, I check a second test before making the diagnosis. If the person has clear symptoms of high sugar, such as increased thirst, frequent urination, or unexplained weight loss, one high test together with symptoms can be enough to start treatment.
Earlier in my career the standard advice was different. The guidance moved, and so did we. I will be honest, we do not fully understand why this reaches some people earlier than others, and that is why I combine the lab number with the clinical picture and home readings before deciding.

Can HbA1c be high if fasting sugar is normal?
Yes. That is one of the most common frustrations I hear: “My fasting sugar is okay, so why is my HbA1c bad?” The reason is post-meal spikes. If your fasting numbers are fine but your blood sugar rises high after eating, those repeated spikes lift the average even though mornings look normal.
Nine times out of ten the real issue turns out to be something simpler than you expect. Skipping the evening walk, a single heavy meal at a family dawat, or frequent chai and biscuit rounds at the office can all nudge the average upward. For people who check only fasting values, those after-meal rises stay hidden unless we ask for post-meal checks or a short home glucose log.
How often should HbA1c be checked?
Because red blood cells live about 2 to 3 months, HbA1c shows a delayed picture of your control. For that reason I usually check HbA1c every three months when I am starting or changing treatment and every six months once things are stable. If you change diet or medication, expect the full effect to show up only after a few months.
Patients rarely believe me the first time I say this, but testing too soon after a change often gives a false sense of progress or failure. If you started a new plan last week, the next HbA1c will not tell the whole story yet.

When I want to see you
Below I list the situations that make me ask for an appointment in the next week, and the signs that need urgent care. If you have both a lab report and home readings, bring them. The list that follows is what I mean by “book soon” and “go to emergency now.”
- Book an appointment soon if you have a new HbA1c in the diabetes range but feel well, your fasting sugars and post-meal logs disagree with the lab, or your HbA1c has risen sharply since the last test.
- Go to emergency now if your home readings are very high and you are vomiting, confused, breathing fast, or severely dehydrated. If your reading crosses 300 and you are vomiting or confused, get urgent care without delay.
What to write down before your appointment
When you come to me, the most useful thing is a short, honest record. By the time someone reaches me with this, they have usually already tried three things at home. I want the real data, not the story you wish had happened.
- Two weeks of home readings: fasting and one or two post-meal checks, noting the time after the meal
- Any new medicines, including over-the-counter herbs or a weight-loss injection if you are taking one under guidance
- Any symptoms such as thirst, urination, dizziness, or episodes of low sugar
- Notes about recent illness, blood loss, transfusion, or pregnancy if applicable
- Any questions about Ramadan timing, sehri and iftar changes, or long drives where medicines were missed

Bring the right reports to your visit
Bring the HbA1c report itself and any recent fingerstick logs. If you have been to another clinic, bring those reports too. There are a few conditions that can change how I read the HbA1c: pregnancy, anaemia, recent heavy bleeding or transfusion, and advanced kidney disease. Tell me about them because they can make the percentage misleading.
I will be honest, we do not fully understand why some numbers and some people do not match perfectly. When the HbA1c and home readings disagree, I look for altered red cell turnover or the pattern of post-meal spikes. That directs what we check next.
Questions patients ask me after an HbA1c result
How can I reduce my HbA1c?
Start with smallest sustainable changes. If you can only change one meal, change the one you eat with family. Regular walking after the main meal, cutting down repeated snacks like biscuits with chai, and measuring portion sizes of roti and rice usually move the number over months. If medicines are needed, I start low and adjust, and never stop insulin without your doctor.
What does HbA1c tell you?
It gives a 2 to 3 month average of blood glucose. Think of it as the lab’s memory of how much sugar was in your blood during that period. It does not replace your meter for day to day decisions.
Is 6.5 HbA1c diabetes?
A 6.5% result is in the diabetes range. Often I repeat the test to confirm if the person has no symptoms. If symptoms are present, we treat based on the combination of the number and clinical signs.
Can HbA1c be high if fasting sugar is normal?
Yes. After-meal spikes can raise the average even when mornings look okay. A short home log of post-meal checks usually reveals the pattern.
How long before a diet change shows on HbA1c?
You need a few months to see the full effect. I tell patients not to expect a major drop after one week. The lab averages the last 2 to 3 months, so changes appear gradually.
Most of my patients are surprised when I tell them that ongoing small habits beat a dramatic short-term push. Fear is a worse motivator than habit, and habit is what I am after with you. If your plan depends on you being disciplined every single day, I do not trust it and neither should you.
Bring your reports and the short log
Bring the HbA1c report, two weeks of meter readings, and a short list of symptoms or missed medicines. That will let me match the lab number to your daily life and decide whether we need a repeat test, changes to food and activity, or a medication review. Dr. Rizwan Niazi assesses these cases personally and will review your records with you.
There are a few practical things I see go wrong. Load shedding in summer can make people miss insulin storage steps. A long drive to a family function sometimes means medicines are left behind. The meal after taraweeh can be much larger than patients expect. These are ordinary problems with ordinary fixes. I have watched this go wrong the same way many times, which is why I ask about these details up front.
